When you submit a consultation request or contact form, we collect the information you provide directly, such as your name, work email, company, and message content. We do not collect individual employee health information as part of our organizational intake process.
Information submitted through our forms is used solely to respond to your inquiry and, where applicable, to prepare a program proposal for your organization. We do not sell or share your information with third parties for marketing purposes.
Where MindShift Wellbeing coordinates access to licensed clinicians on behalf of a client organization, individual employee participation and clinical details remain confidential and are not disclosed to the employer. Aggregate, de-identified utilization data may be shared with HR sponsors as outlined in individual service agreements.
Contact form submissions are retained only as long as necessary to respond to inquiries and maintain business records, consistent with applicable law.
